  • FAIL: critical or major failures, including security vulnerabilities
  • CONDITIONAL: minor/trivial failures, quarantined or inconclusive checks, or requirements coverage below 80%
  • PASS: every executed check passes on the first attempt
  • Disabled checks show “Unavailable,” not zero
